Dementia Evaluation

A dementia evaluation is a structured process designed to answer three questions: is there a measurable change in cognition, what is the most likely cause, and what can be treated or supported.

What the visit generally includes

  • Detailed history of the memory or thinking changes over time
  • Input from a family member or close friend when possible
  • Review of medications, sleep, mood, and other medical conditions
  • Neurological examination
  • Cognitive and memory testing
  • Laboratory studies
  • Brain imaging when appropriate
  • Additional testing, including biomarkers, when clinically indicated

How to prepare

  • Bring a complete list of medications and supplements
  • Bring prior records, imaging reports, and laboratory results if available
  • Bring a family member or friend who has observed the changes
  • Write down specific examples and when they began

What you should leave with

You should understand what may be causing the symptoms, why any testing is being performed, and what the plan is going forward.
