When should migraine be evaluated by a neurologist?
Short answer
Migraine should be evaluated by a neurologist when headaches are frequent, when they are not adequately controlled, when the pattern changes, when acute medications are needed often, or when new neurological symptoms accompany the headaches.
Understanding this
Frequent headache days and frequent use of acute medication can lead to a cycle in which headaches become more frequent. Recognizing this early changes the treatment approach.
A change in an established headache pattern is worth attention, as is a headache that begins after age 50, worsens with position or straining, or comes with fever, neurological symptoms, or vision change.
Symptoms and signs
- Moderate to severe, often one-sided, throbbing headache
- Nausea or vomiting
- Sensitivity to light, sound, or smells
- Visual or sensory aura before or during headache
- Headache worsened by activity
- Headaches on 15 or more days per month
How it is evaluated
- Detailed headache history and pattern review
- Review of current and prior treatments
- Neurological examination
- Imaging when clinically indicated
- Discussion of acute and preventive treatment options
When to seek care
- Seek emergency care for a sudden, severe headache that peaks within seconds to minutes, or a headache with fever and stiff neck, confusion, weakness, difficulty speaking, or vision loss.
- Seek prompt care for a new headache after head injury or a distinctly different headache pattern.
